Please read these Terms and Conditions of Use ("Terms") carefully before using this website (the "Site") or the services made available through it. By accessing or using the Site, you acknowledge that you have read, understood, and agree to be bound by these Terms. If you do not agree to these Terms, you may not access or use the Site.
The Site is operated by CarePrime Men LLC ("CarePrime Men," "we," "us," or "our"). CarePrime Men reserves the right to revise these Terms at any time. Revisions are effective upon posting to the Site, and your continued use of the Site after a revision is posted constitutes acceptance of the revised Terms. You should review this page periodically.
The Site and the Services are not intended for use in a medical emergency. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
CarePrime Men provides telehealth services in men's health focused on hormone and wellness therapy through Christopher Brown, FNP, a board-certified family nurse practitioner (the "Provider"). The Provider may offer in-person visits as well, at locations to be determined by the Provider. In states that grant nurse practitioners full practice authority, the Provider practices independently within his scope of practice. In states that do not grant full practice authority, such as Georgia as of the Effective Date of these Terms, the Provider practices under the supervision of a licensed physician, currently Caleb Brown, M.D., which supervising physician may change from time to time. Services currently include: (a) visits with the Provider, through telehealth and/or in person as permitted and appropriate; (b) laboratory testing coordinated through third-party laboratories; and (c) prescription medications, where clinically appropriate, dispensed and shipped by third-party pharmacies (collectively, the "Services").
Services are currently available only to patients located in Georgia. CarePrime Men may expand the states in which Services are offered as the Provider becomes licensed in additional states, and may discontinue availability in any state at its discretion. It is your responsibility to confirm that the Services are available in the state where you are physically located at the time of your visit.
Content on the Site describing health and wellness topics is provided for general informational purposes only. It is not personalized medical advice and does not create a provider-patient relationship. A provider-patient relationship is established only after you complete registration, an intake process, and an initial visit with the Provider.
To use the Site or the Services, you represent and warrant that:
Meeting these requirements does not guarantee that you will be accepted as a patient or that any particular treatment will be prescribed. The Provider will independently determine, in the exercise of clinical judgment, whether a treatment is appropriate for you.
CarePrime Men operates on a cash-pay, subscription basis and does not bill health insurance. The specific services, labs, and/or medications included in your subscription depend on the particular package or subscription agreement you select at the time of enrollment, which will describe what is included in that package. Available packages, and what they include, may change from time to time; the terms of the specific package or agreement you sign up for will govern what your subscription fee covers.
YOUR SUBSCRIPTION AUTOMATICALLY RENEWS EACH MONTH UNTIL YOU CANCEL. Your payment method on file will be charged automatically at the start of each billing cycle unless you cancel before the renewal date. You may cancel your subscription at any time by contacting us by phone at 912-298-2957 or by submitting a cancellation request through the patient portal; cancellation will be effective at the end of your then-current billing cycle, and you will not be charged for the following cycle.
Because your subscription fee reflects the cost of the services, labs, and/or medications included in your selected package or subscription agreement, refunds are not provided for any partial billing period or for services, labs, or medications already rendered, ordered, or shipped as of your cancellation date, except as required by law. If a lab order or prescription has already been submitted to a third-party laboratory or pharmacy at the time you cancel, you remain responsible for the corresponding portion of that billing cycle's fee.
Your subscription is billed through our patient portal (hosted by RXNT), which currently uses PAYA, a third-party payment processor, to process your payment. CarePrime Men may use a different or additional billing or payment processing arrangement in the future. Certain third-party services referenced in these Terms, such as Fullscript, may separately collect payment information directly from you for products or services purchased through their own platforms; any such transactions are governed by that third party's own terms, not by these Terms. CarePrime Men uses reasonable measures designed to protect any payment information it processes, consistent with applicable law.
Where the Provider determines a prescription is clinically appropriate, prescriptions are sent to a third-party pharmacy for compounding or dispensing and direct shipment to you. As of the Effective Date, CarePrime Men works with Empower Pharmacy, MFV Compounding, and ReviveRX Pharmacy, and may add or change pharmacy partners from time to time. Pharmacies are independent third parties, not owned or operated by CarePrime Men, and are solely responsible for the accuracy of compounding, packaging, and shipment of your medication.
Nothing in these Terms guarantees that any particular medication will be prescribed, that any prescription will be approved or filled by a pharmacy, or that insurance (if applicable to a lab test) will cover any portion of your care.
Where lab testing is part of your treatment plan, CarePrime Men coordinates testing through third-party laboratories, which may include Labcorp and Quest Diagnostics. These laboratories are independent of CarePrime Men and are responsible for specimen collection logistics, processing, and result accuracy consistent with their own policies.
The Site provides a link to our patient portal, hosted by RXNT, where you may schedule appointments, communicate with the Provider, and submit payment. Your use of the patient portal is governed by RXNT's own terms of use, and any health information you submit there is governed by our Notice of Privacy Practices and RXNT's own privacy policies, not by these Terms.
The Site may also link to or reference Fullscript, a third-party platform for supplement recommendations and purchases. CarePrime Men does not control, and is not responsible for, the content, products, privacy practices, or terms of any third-party site or platform, including RXNT, Fullscript, Labcorp, Quest Diagnostics, or any pharmacy. Your use of any such third-party service is at your own risk and subject to that website's own terms.
All content on the Site, including text, graphics, logos, and images, is owned by or licensed to CarePrime Men and is protected by copyright, trademark, and other applicable laws. You may view and print Site content for your own personal, non-commercial use. No other use of Site content is permitted without our prior written permission.
THE SITE AND SERVICES ARE PROVIDED "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. CAREPRIME MEN DOES NOT WARRANT THAT THE SITE WILL BE UNINTERRUPTED OR ERROR-FREE, THAT DEFECTS WILL BE CORRECTED, OR THAT ANY MEDICATION SHIPMENT WILL ARRIVE ON A PARTICULAR SCHEDULE OR IN A PARTICULAR CONDITION UPON DELIVERY BY A THIRD-PARTY PHARMACY OR CARRIER.
TO THE FULLEST EXTENT PERMITTED BY LAW, CAREPRIME MEN, ITS OWNERS, EMPLOYEES, AND CONTRACTORS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, PUNITIVE, OR CONSEQUENTIAL DAMAGES ARISING FROM YOUR USE OF THE SITE OR SERVICES, OR FROM ANY ACT, OMISSION, DELAY, OR ERROR OF ANY THIRD-PARTY PHARMACY, LABORATORY, CARRIER, OR PLATFORM (INCLUDING BUT NOT LIMITED TO RXNT, FULLSCRIPT, LABCORP, QUEST DIAGNOSTICS, EMPOWER PHARMACY, MFV COMPOUNDING, OR REVIVERX PHARMACY), EVEN IF CAREPRIME MEN HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
You agree to indemnify and hold harmless CarePrime Men and its owners, employees, and contractors from any claims, losses, or expenses (including reasonable attorneys' fees) arising from your misuse of the Site, your violation of these Terms, or your provision of false or misleading information.
These Terms are governed by the laws of the State of Georgia, without regard to its conflict of laws principles. You agree that any dispute arising out of or relating to these Terms or the Site will be brought exclusively in the state or federal courts located in (or the nearest court with jurisdiction over) Glynn County, Georgia, and you consent to the personal jurisdiction of those courts.
If any provision of these Terms is found unenforceable, the remaining provisions will remain in full effect. These Terms, together with our Privacy Policy and Notice of Privacy Practices, constitute the entire agreement between you and CarePrime Men regarding the Site. Our failure to enforce any provision is not a waiver of our right to do so later.
